Jason Kravetz | Director of Case Management Services

Jason is currently the Director of Case Management services at Charak Center for Health and Wellness. He is married and has a 5 year old son. He enjoys running and Cleveland sports in his spare time. His educational background is a Bachelor of Arts in Psychology and a Masters of Education degree in School Counseling, both from Kent State University. His educational background is primarily in the school setting, however his work experience has been almost exclusively in the community setting. As far as work experience, he has done case management work for the past 8 and a half years. In November of 2014, he was promoted to the Director of Case Management at Charak and has been working in the role since. Since assuming responsibilities of this role, the growth of the case management services at each site has increased and they are now working to increase overall awareness of the service within the communities they serve.

Rachel Astorino | Case Manager

Rachel has a bachelor's degree in Psychology and a minor in Women's, Gender, and Sexuality Studies.

Veronica Kelly | Case Manager  

Veronica is a case manager at the Mentor office. She received her Bachelor of Arts Degree in Human Resource Development with a minor in Psychology, from Notre Dame College of Ohio. She received her Masters of Science in Elementary Education, from Kaplan University specializing in Language and Literacy. She has over 25+ years of case management experience.

Lishawn Burton | Transportation Manager

Lishawn started at Charak in 2011. She has worked with PHP and IOP clients, and helps coordinate transportation for patients in groups and with appointments.

Lishawn is located at the Garfield office. Ext. 3565.

Vada Williams | Case Manager

Vada is a certified medical assistant and she's worked with clients in group homes before coming to Charak. She has 10 years experience working with clients with low functioning mental illnesses.

LeeAnn Parker| LSW/Administrative Director of Clinical Case Management

LeeAnn has her bachelor’s degree from Youngstown State University in Social Work and is a Licensed Social Worker. She assists Jason Kravetz with services and high-risk clients, as well as does diagnostic assessments and case management.

Melinda Belfiore | LSW/ High Risk Case Manager

Melinda has a License in Social Work and graduated from Cleveland State University. She has a background in Developmental Disabilities, mental health, hospice, prader-willi syndrome, sex offenders, & gang members. She has performed duties such as basic vitals and job coaching, case managing for foster and adoption, a Youth and family worker, and a Workshop Specialist for medically fragile developmental disabilities. She is working out of the Medina office.

Nichole Kopp | Case Manager

Nichole worked as the front desk assistant in Elyria for a year before becoming a case manager. Her extension is 114.

 

 

 

Krystal Cortes | Case Manager

Krystal has her Bachelors of Health Science from Cleveland State University. Her previous work history includes working as a caregiver and a direct support professional where she cared for people with Alzheimer's and dementia as well as developmental disabilities. She currently works as a case manager out of the Elyria office and extension is 117.

 

Dymond Caver | Case Manager

Dymond has her Associate of Arts degree from Lorain County Community College and a B.A. with a major in Psychology and minor in Sociology from the University of Dayton. She also has a M.S. in Criminal Justice with a concentration in Forensic Psychology from Tiffin University.  She previously worked as a victim advocate for Domestic Violence victims. Dymond is a case manager in the Elyria office.

 

Marva Newborn-Walls | Case Manager

Marva started college at the age of 31 attending Cuyahoga Community College, where she obtained an Associate of Arts Degree. 

She then furthered her career by her enrollment into Cleveland State University where she obtained a Bachelor of Arts Degree, specializing in Social Work. 

She chose Social Work as her profession because she was once on the receiving end of needing the same services as her client base. Marva is a case manager in the Garfield office.

Heather Simon | Case Manager

Heather is working on her bachelor's degree in psychology with a minor in sociology. Heather is a case manager in the Mentor office.

Danelle Wilson | Case Manager

Danelle has a bachelor's degree in psychology. She graduated from Hiram College in 2012. Danelle is a case manager in the Garfield office.

Bethany Longstaff | Case Manager

Bethany attended Bowling Green State University where she acquired her Bachelor’s Degree in Psychology with a minor in Sociology. She is currently studying to obtain her Masters of Science in Mental Health Counseling. Bethany is a case manager at the Garfield Heights Location.

Shannon Brown | Case Manager

Shannon attended The University of Akron where she acquired her Bachelor’s Degree in Social Work. She is currently studying to obtain her Masters of Science in Clinical Social Work. Shannon is a case manager at the Garfield office.

Elysia McNutt | Case Manager

Elysia is from Ashtabula. She is currently attending Lakeland Community College. Elysia is a case manager at the Mentor location.

Nicollette Pritts | Case Manager

Nicollette has a Bachelor of Science in Health and Human Services from Kent State University. She has a background with individuals with developmental disabilities. Nicollette is a case manager at the Stow office. 

Caitlin Green | Case Manager

Caitlin graduated from Tiffin Univeristy in 2017 with a bachelor's degree in Forensic Psychology and a minor in Forensic Science. She is currently working toward earning her master's degree in Psychology from Tiffin University as well. She has a background in mental health and substance use in teens, earning both her CDCA and QMHS. She is a case manager at the Medina location.

Jeremy Davis | Case Manager

Before becoming a case manager Jeremy spent six years assisting the mentally disabled. He has a Bachelor of Arts in Human Services. He is currently attending Youngstown State University for his Masters in Social Work . Jeremy is a case manager at the Garfield Heights location.

Heather Younkin | Case Manager

Heather is a former student-athlete at Kent State University where she graduated with a Bachelor's of Science in Public Health and a minor in Community Health Education. She is currently pursuing a Masters of Social Work at The University of Akron. Heather is a case manager at the Elyria office.

Evan Acri | Case Manager

Evan earned his Bachelor of Arts in Psychology from Bowling Green State University in 2015. He is a case manager at the Garfield Heights location.

 

 

 

Shana Thomas | Case Manager

Shana has her Associates of Science Degree from Kent State University. She is currently a senior, finishing up her Bachelor's degree in Human Development and Family Studies and minoring in Psychology. She then hopes to go on to grad school to become a counselor. Shana is at the Stow office.

 

 

 

Analia Colon | Case Manager

Analia has her associates of Arts degree and has her CDCA. She works as a case manager but also does assessments and AOD therapy. Analia works out of the Elyria office.
